      Smile Train's vision is that every child born with a cleft – anywhere in the world – will have the opportunity to live a full and productive life. Safety and quality remain our top priorities and Smile Train is committed to ensuring that no child’s life is put to an unacceptable level of risk for an elective procedure and that the surgical treatment provided is of acceptable quality. The following guidelines are for partners approved to perform pharyngeal flap and pharyngoplasty procedures.

      Facilities:
       
  
      設備：
       
  
      ·         Must have dedicated operating rooms available on scheduled surgery dates to minimize cancellations.
       
  
      ·         必須擁有專用的手術間，可以使手術在規(guī)定時間內(nèi)進行，盡量減少取消手術。
       
  
      ·         Must have a recovery room with appropriate monitoring equipment. 
       
  
      ·         必須擁有具備適當監(jiān)護設備的麻醉蘇醒間。
       
  
      ·         Must have post-operative wards with adequate number of qualified nurses on the ward 24 hours/day and appropriate monitoring equipment (minimum – pulse oximeters for all post-op patients).
       
  
      ·         必須擁有術后恢復病房，病房每天24小時都有足夠多有資質(zhì)的護士并且配備適當?shù)谋O(jiān)護設備。（至少所有術后患者都有脈搏氧飽和儀）
       
  
      ·         Must have dedicated staff for careful and frequent observation in the recovery ward.
       
  
      ·         必須具有專業(yè)人員可以在蘇醒間仔細并隨時觀察病人。
       
  
       
  
      Surgeons:
       
  
      手術醫(yī)生：
       
  
      ·         Must have adequate training/experience, with an on-going cleft practice, and documentation of at least 10 such procedures performed in the past year with no significant untoward sequelae.
       
  
      ·         必須具有較強的臨床能力和足夠的臨床經(jīng)驗，一直從事唇腭裂臨床工作，過去1年具有至少10例此類手術的病案記載，并沒有造成嚴重的難治后遺癥。
       
  
      ·         Must be personally responsible for surgery. 
       
  
      ·         必須親自主刀手術。
       
  
      ·         Must understand how to explore and respond to undue bleeding.
       
  
      ·         必須明白如何探查并處理大出血。
       
  
      ·         Must have access to magnifying glasses or surgical loupes. 
       
  
      ·         必須具有放大眼鏡或小型外科放大鏡。
       
  
       
  
      Anesthesiologists/Nurse Anesthetists:
       
  
      麻醉師/麻醉護士：
       
  
      ·         Must have adequate training/experience with facilities to reintubate a patient on the floor and to provide care for an intubated patient outside of the operating room.
       
  
      ·         具有較強的能力和足夠的臨床經(jīng)驗可以使用設備在地板上進行再插管，并可以為手術室外插管的病人提供相應的治療
       
  
      ·         Must have adequate experience in the administration of paediatric anaesthesia. 
       
  
      ·         必須具有足夠的實施小兒麻醉的經(jīng)驗。
       
  
      ·         Must know how to perform a tracheostomy for significant post-op airway problems.
       
  
      ·         必須知道如何對術后有嚴重呼吸問題的病人實行氣管切開術。
       
  
       
  
      Cleft Team:

      ·         Should include speech pathologist, audiologist, and otolaryngologist experienced in cleft care.

      ·         Should meet regularly to discuss surgical outcomes.

      ·         Should have close liaison with paediatricians.
